Dear Parents/Guardians,
I am reaching out to you to ask for your help in completing a brief online survey, which I have provided the link to below. This online survey is known as The 5 Essentials Survey. The 5 Essentials Survey provides school districts with important information and feedback from students, parents and teachers on how to improve the school and help increase our students’ achievement. The results from the survey will also be published on the Illinois State Report Card.
According to the research provided to us, information from the 5 Essentials for School Success
has shown that schools that meet 3 of the 5 Essentials are 10 times more likely to improve
student success, attendance, and achievement. Thank you for taking your valuable time to
complete this survey so that we can continue to work together to provide the best educational
opportunities for your students. The survey will be open from January 11, 2022, until March 11, 2022. Thank you again for your help with this matter.
